Parents/Guardians and Students,

When planning for a college visit, fill out this form PRIOR to college visit (if it is to be used for the attendance review process).

Before the visit: 

• Download the “College Visitation Verification (CVV) Form” (link) or pick up a paper copy in Room 1019.
• Fill out part one (1)
• Have the Senior Administrator sign the CVV Form (Ms. Vica Irving – Class of 2023)

During the visit:

• Have a college Official complete and sign Part Two (2) of the CVV Form.

After the visit:

•  Take the signed CVV Form to our Attendance Office to be used in the attendance review process.
